Most people think hydration is drinking
plain water…
-but, that can make things WORSE
water dilutes blood sodium levels,
your body wants to excrete it…
lowers blood volume!
This raises susceptibility to cramps and spasms
(dilutes electrolytes)
"It is actually one of the worst things to do…
drinking plain water prior to work out performance"

When doing vigorous exercise
(64% + VO2 max)
-fluids emptying from stomach into intestine
goes down
Example: drinking 5 oz of water every 15 min
EXCEEDS your stomach's ability to empty…
-leading to bloating and cramping
-decreases vigorous exercise performance by 2.5%
Marathon running…when over consuming plain water

Basic ways to hydrate…
Dehydration-acclimation (for improved

-my patients would say:
"feel terrible"
-would pass out
(it bottoms out your blood VOLUME)
-'Low Salt Guidelines' were not matching the results of patients
-patients, following doctor's updated advice, added salt back in…
~the instantly felt much better…even with atrial fibrillation:
-their episodes would almost all go away

Evidence supporting low-salt guidelines…
-Level 'C', just expert OPINION
-not human randomized, control trials
Research showed:
-not just that evidence did not suggest
low salt was the correct advice…
"Looking at real world population,
there is so much harm you can see"

20% of people with normal blood pressure
are 'salt sensitive'
(will see significant reduction in blood pressure)
"I think reducing salt intake is harmful…
you are decreasing blood volume & are

To lower blood pressure:
-improve insulin sensitivity
-increase dilation of arteries and
-increase nitric oxide

When you reduce sodium to 3000mg/day…

Visible signs of insulin resistance in fat cells…
-acanthosis nigricans
Large, rough sections of 'tissue paper' like
skin near skins folds, especially around the
neck and arm pits
-skin tags, in same areas, little projection of
skin, like a small mushroom stalk
Almost proof positive that that person has insulin resistance

The reason: excess insulin…
-the insulin is over-stimulating the skin cells

Question: what amount of ketone supplement
to take?
-no data to quantify the amount, but…
-for ketone esters, around 50 to 100mL
~will bring user to strong ketosis
~would normally take 36 hours of fasting
Ketone salt: daily amount would get only to one tenth as much
"No question ketone esters are more effective,
but also much more expensive"

There is evidence that, in rodents, ketones
-enhance recovery from traumatic brain injury
When someone has experienced
traumatic brain injury…
-they are given lactate and ketones,
~both provide fuel for the brain &
~enhance recovery
Part of what has happened is:
-damage to an enzyme that enhances glucose burning
(pyruvate dehydrogenase) gets compromised with
traumatic brain injury…so "any other fuel would become therapeutic"
~like ketones or lactate

Lactate has been erroneously vilified…
-humans never have 'lactic acid'
-lactate is a molecule not related to acidity
-it has nothing to do with muscle soreness
-lactate is made by a working muscle, then
-the liver takes it in and returns it to the
muscles as glucose
-brain can pull lactate in as a fuel in modest amounts
-glucose & ketones are primary; lactate is tertiary brain fuel
