-carbs
-it was not until the age of agriculture and eating grains
that the age plummeted
"The more carbs eaten, the earlier a girl will menstruate”
-hunter-gatherers: 17, 18 years
-since then the age has lowered
-including formation of hips
-giving birth became much more
dangerous and painful

High carb raises insulin levels & cortisol, a stimulant, all
throwing a wrench into delicate endocrine system…
-our endocrine system is designed to eat meat & fat
-full stop
-when our ancestors ran out of meat & fat, we survived
by eating some berries
-by fermenting, vegetables are
more palatable
Scary to learn what happened to
our bodies since advent of agriculture

"My youngest daughter had an accident at 18, almost
died. Her adrenals kept her alive.
-I had to treat her adrenals for several years
-she had never been hypothyroid
-when pregnant, I insisted they do more free T3; T4 tests
-she was indeed hypothyroid
"It was due to her shock during
car accident that caused her
to become hypothyroid in pregnancy”
-will treat thyroid all through nursing

Stress response can trigger low thyroid function…
-trauma is very nutritionally expensive
"It is all about nutrients.
-increase the nutrients, reduce the stress: will be fine”

Other factors, besides diet, that affect our hormones?
-bromide in our water, in our couch fabrics
-PFAs; toxins; plastic bottles
So many plants are problematic
-teas; mint; broccoli; we simply are not designed to eat
If eating strong carnivore diet, then
having some of these plants won’t
do damage
If you have an issue, who to fix it
without removing those toxins?

Anti-depressants like Prozac, lowers free T3 and Free T4…
-this messes with cortisol levels
-papers say, while taking meds, cortisol is down, but
what about later?
-there will be a negative feedback response later
All these meds lower thyroid
It is growth hormone
-meds negate sympathetic nervous
system, but DON'T ADDRESS
ROOT CAUSE ITSELF

Before birth there is a lot of hormonal change in the
fetus : cortisol, the androgens secreted in huge amounts
for growth, brain function…
-surge in hormones until birth, then they go down until
adolescence
-puberty is a birth; changing from
a child into a fertile being
-critical to be healthy and capable
of having healthy offspring

If something is missing, there will be negative feedback
response to bring back balance…
-females were designed, evolved to be fertile beings at
17, not 8, 9 or 10
-menstruation indicates: now fertile…are we ready to
become pregnant at 8? No
-brain is neuroplastic until age 28
Puberty is developing sexual
characteristics; adolescence
continues until about age 28

A lot of hormonal signaling going on, all triggered by
thyroid hormone…
-thyroid is conductor of the orchestra making a child into
a fertile being capable of offspring

Question: impact of synthetic hormones during this time?
-brain does not recognize them as such…
-steroid hormone cascade starts as cholesterol, in both
ovaries & adrenals, then turns into different hormones
Beginning of cascade: stress response
Ending: testosterone & estradiol
-for success, need to have balance

Synthetic hormones are not replacing testosterone &
estrogen…they are raising cortisol levels
-which absolutely has an effect on the brain
Example: birth control–hormones that raise cortisol?
-yes, and put a wrench in this delicate net:
-HPA access & HPT access
-throws everything off
-causes a stress response in the
system

Cortisol affects amygdala formation & cortex formation
-many women say this made me feel better when taking
the hormones is because they raised the cortisol levels
So, giving 8 year old, due to her ADHD, hormones…
supposing they will modulate cognitive function.

high carb, medium carb, low carb
Result: low carb better initially and long term
More: When IR, and on high carb, did much worse

Carb intolerance: many factors:

being oxidized, used as fuel in muscle
-when carb intolerant, significant portion of carb intake
being converted to body fat in

-carb intolerance is mismanagement

-predictive of higher TG; increase in

If reasonably carb tolerant, can begin increasing carbs
and monitor these biomarkers
-TG; sdLDL; HDL; glucose
For carb tolerant, upper end of carbs / day 100-150g
-if body weight is not excessive
-if they had carb intolerance before
Protein: "for me, 100-150g / day”
So, for 75kg [165 pound] man:
carbs + protein = 1000 cal
fat needs to be 1500 cal

What is best diet?
-people vary from one to another
-those with symptoms of IR will need ultra low carb
intake to get into keto adaptation
-body will produce ketones
For most in this situation, under
50g carbs a day does it
-others may need to go lower

"For some, 25g a day is needed to get into ketosis
fuel system, others, who are carb tolerant, can eat up
to 100g carbs a day and still get benefits of ketosis”
-diet must be individualized to find sweet spot for each
person

In my 30s, 40s I could tolerate vast amounts of carbs…
-at 50, gained fat in middle, higher BP
-following popluar advice at the time:
-lots of fruits & veggies

-APOE4 gene found on 19th chromosome, comes in 3
flavors: APOE2, APOE3, APOE4
-everyone gets one from each parent, and if you get
both APOE4, have strongest 5X risk of Alzheimers
"Genetics is not fate”
But whether you get it or not
is dependent upon your
metabolic health

Those in good metabolic health, regardless of their
APOE4 status, do NOT have increased risk of Alzheimers
-the APOE gene serves as blueprint for this protein
which is embedded within the membrane of HDL
cholesterol particles
-HDL in brain act as a
Natural Defense against
beta amyloid plaque
deposition

HDL particles in brain actually Remove beta amyloid…
-APOE3 variant is by far most common
-less common APOE4 gets all the bad press
it is especially susceptible to damage due to its unique
molecular structure
Beta amyloid peptides can be
damaged by sugar stress &
oxidative stress from intake of
oxidized seed oils

APOE protein can also be damaged in the same ways…
-APOE4, due to molecular structure, is especially prone
Clear evidence, from this paper, comparing APOE3
tendency to form advanced glycated end products to
APOE4 protein:
APOE4 is damaged
at 3X that of APOE3

If you carry APOE4, and are not metabolically healthy,
HDL particles in your brain, which remove toxic protein
deposits, are less likely to do that
-this tendency for glycation damage when not healthy

Consider: 98% of US adults, over 60, are not completely

matrix metalloproteinase 9 has been shown to interrupt
the blood-brain barrier…which prevents certain
circulating substances, in our blood, from crossing into

early biomarker of
Alzheimers

Paper from last year, in Nature: increases in blood-brain
barrier dysfunction, caused by high matrix
metalloproteinase 9 levels, were higher in those carrying
the APOE4 protein…making their HDL prone to damage

-all contained oxidative products—and
-38% failing the voluntary industry standard
-perhaps that is why vast majority of fish oil studies find
No Benefit
-16 of 18 studies
showed no advantage

When we consume oxidized oils, we absorb the

Study of 2 genetically identical populations in
Indianapolis, USA and a city in Nigeria, Africa—which
were historically linked by the slave trade…
Findings:
USA only 2% are

-Alzheimers: 2.5X more than

Same holds true for those carrying the ApoE4 gene, also
called the Alzheimers gene…

Observational study: why is obesity apparently
protective in younger women breast cancer vs harmful
in postmenopausal women?
When younger, ups and downs in insulin, but fat cells
still take up fat easily–don't need excess levels of insulin;
fat cells still insulin sensitive, so insulin
drops down as usual
After decades, cells are IR: so insulin
must rise constantly

Typical cancer physicians do not understand the
metabolic implications…
'Fat is good, then it is bad?’
No: excess insulin is the wrong path from the beginning
-tend to be more IR as we age
-today: this starts earlier and earlier
For many: breast cancer from IR is
mid-life disease

Question: how often to get blood work test?
-once a year, fasting labs
Check insulin & glucose; and the interactions between
-fasting TG levels: if both TG & glucose high: big problem
TG/HDL ratio to check for IR [if less than 1.5 okay]
If any of these 3 are abnormal according
to my range, then we know: some IR

If all three, insulin, glucose, TG are normal, but
patient has between meal challenges, hangry, depression,
or if they really cannot skip a meal–that is a sign of
significant IR
-in that case, insulin is still ON and pushing down glucose
Some young patients have fasting labs
that look okay…but, check on
-brain fog, fatigue…
probably something hiding between
the fasting numbers

Some cancer patients need to focus on getting in
enough fat calories–due to chemo symptoms taking
away appetite or making one nauseous…
Utilize days when feeling good to build back up mass
A common mistake when starting keto is to just
take out carbs from diet but not adding
in animal fats, so:
-no energy
Must get energy from animal fat:
whole eggs; fatty meats…

Healing requires more amino acids, building blocks to
help repair–cancer therapy is constant repair process…
Must heal between each chemo therapy cycle &
between each dose of radiation
"Schedule when to eat. It is your job to rebuild”

In beginning I was dealing with breast & endometrial
cancers–very highly associated with obesity…
-so employed low carb to help lose weight
Data shows, even if obese or normal weight—
it is not the weight, it is the insulin
-if obese, your insulin is too high
-may look normal after fasting, but
being overweight means insulin is
actively storing fat
